Community Nurse Job Vacancy, Wembley

Chase Medical is currently recruiting for an exciting new Enhanced Care Service, which will be based in Wembley and surrounding areas (London Borough of Brent). This is an opportunity to work closely with the CCG on a mission service visiting Care Homes in the area. As the successful candidate you will be working with a team of GPs from a local hub/practice.
- The hours on offer are full time however a job share would be considered.
- Working hours are flexible within the 7 day service, which operates 8am to 8pm daily. So we can build your rota around your commitments, with evenings and weekends being optional.
- Salary is negotiable and dependent on experience - band 6 and 7 nurses will be considered.
- Duties of the role will include: Medication Reviews, Ward Rounds, Telephone Triage and Community Visits to reduce Ambulance Requests.
This is a fantastic opportunity to take a step into Primary Care, and boost your CPD as a Community Nurse. The contracts are currently temporary however likely to be extended to permanent for anyone who wishes to move their career forward with the organisation.
